All physicals and athletic permits need to be scanned and emailed to [email protected] as soon as possible.

Please make sure they are completely filled out and signed by the physician.

Please Note: This registration does not guarantee that your student-athlete will make the team! ONCE it is submitted and the athletic director has verified grades and received a current physical exam form, the student athlete will be cleared to try-out for the specified team.
